Job Overview
This position is for a Physical Therapist (PT) in a home health setting, providing care to patients in the comfort of their own homes. The role involves delivering physical therapy services as prescribed by a physician to improve mobility, reduce pain, and enhance overall function. The physical therapist will be responsible for creating and implementing treatment plans, conducting exercises and rehabilitation activities, and documenting patient progress. This position offers flexibility with a hybrid schedule, covering a designated service area. The role also includes working collaboratively with other healthcare professionals and participating in ongoing education and training to ensure the best possible patient outcomes.
Responsibilities
As a Physical Therapist, your role will include:
- Delivering physical therapy services according to physician's plan of care.
- Treating patients to relieve pain, restore function, and enhance performance.
- Assisting patients with exercises, gait training, and prosthetic training.
- Maintaining clinical records and documentation in a timely manner.
- Participating in team meetings and in-service education programs to enhance skills and knowledge.
Qualifications
- Licensure: Licensed Physical Therapist in California.
- Education: Doctorate or Master’s degree in Physical Therapy.
- Experience: Minimum of one year of PT experience required.
- Certification: Current CPR certification.
- Other: Valid driver’s license, insured vehicle in good working order
