The Hospice Clinical Manager is responsible for overseeing hospice care operations, utilizing their hospice knowledge and leadership skills to ensure high-quality, patient-centered care. Key responsibilities include:
Responsibilities:
• Oversee hospice care operations, using hospice expertise to manage and review patient cases, ensuring care meets high standards of end-of-life services.
• Provide leadership and guidance by mentoring clinicians to promote effective performance and compassionate care delivery. Coordinate closely with the interdisciplinary team and provide real-time support as needed.
• Plan and direct care services through home visits, conferences, and record reviews to assess patient needs and adjust care plans accordingly.
• Support and educate staff by setting immediate and long-term therapeutic goals and offering guidance on hospice care best practices.
• Collaborate with medical and operational teams to coordinate smooth transitions and maintain continuity of care for referred patients.
• Registered nurse with current licensure to practice professional nursing in the state.
• Previous experience in hospice/home care setting and with two (2) years management or supervisory experience. Proven ability to work within an interdisciplinary setting.
• Has excellent observation, good nursing judgment and communication skills. Understands hospice philosophy and issues of death/dying.
