  • Document patient progress, outcomes, and plan updates in the electronic medical record within required timeframes
  • Maintain HIPAA compliance, safety standards, and equipment sanitation on every visit
  • Travel between patient homes within Salem, McMinnville, and Eugene; mileage reimbursement and flexible scheduling provided
  • Eligible for a tiered sign-on bonus based on experience ($5K–$20K)

Qualifications

  • Active Oregon Physical Therapist license (Doctorate or Master’s preferred)
  • Open to new graduates; home-health experience preferred
  • Current CPR/BLS certification
  • Reliable transportation with auto insurance and valid Oregon driver’s license (travel to patient homes required)
  • Strong clinical reasoning, communication, and patient-education skills
  • Ability to work independently, manage a dynamic caseload, and adapt to changing patient needs
  • Eligible for sign-on bonus ranging from $5,000 to $20,000 based on experience
