Responsibilities
- Clinical Leadership: Supervise, mentor, and provide guidance to a multidisciplinary team—including nurses, social workers, chaplains, and home health aides—to foster a collaborative and effective care environment.
- Case Management: Receive case referrals and review available patient information (including home visits) to determine hospice/home care needs. Assign appropriate hospice personnel and confer with the medical director regarding patient eligibility.
- Clinical Evaluation: Evaluate each case using multiple approaches (home visits, conferences, record reviews, and clinician reports); verify impressions, and instruct clinicians to promote effective performance and quality care delivery.
- Patient Care Oversight: Ensure that every patient receives comprehensive, compassionate care that meets their unique needs while promoting comfort and dignity.
- Quality Assurance: Monitor and assess the effectiveness of clinical interventions, ensuring adherence to hospice care protocols, regulatory guidelines, and patient safety standards.
- Care Coordination: Collaborate closely with physicians, caregivers, and the interdisciplinary team to develop, adjust, and implement individualized care plans.
- Compliance and Documentation: Ensure accurate and timely clinical documentation—including assessments, care plans, and progress notes—in compliance with regulatory and hospice standards.
- Training and Education: Provide ongoing education and training for clinical staff on best practices in hospice care, including palliative symptom management, end-of-life issues, and family support.
- Performance Management: Evaluate staff performance, offer constructive feedback, and support continuous professional development.
- Family and Patient Advocacy: Act as a liaison between families and the hospice team, ensuring that the concerns and needs of both patients and their loved ones are addressed.
- On-Call Support: Be available for on-call duties as needed to promptly address clinical issues or emergencies.
Qualifications
- Education: Registered Nurse (RN) with a current, active license in California.
- Experience: Minimum of 2 years in a clinical management role, with at least 1 year in hospice or palliative care. Proven ability to work within an interdisciplinary setting.
- Certification: Hospice and Palliative Care Certification (CHPN) preferred.
- Leadership Skills: Proven ability to lead, motivate, and manage a team of clinical professionals in a fast-paced environment.
- Strong Clinical Knowledge: In-depth understanding of hospice care principles, symptom management, and end-of-life care.
- Excellent Communication: Strong interpersonal, verbal, and written communication skills, with the ability to interact effectively with patients, families, and healthcare teams.
- Problem-Solving Abilities: Excellent critical thinking skills, with the ability to make sound decisions in complex situations.
- Computer Skills: Proficient in electronic medical records (EMR) systems and other clinical software.

Day to Day
As a Hospice Clinical Manager at Mission Healthcare, your day-to-day responsibilities will include overseeing hospice care operations, reviewing patient cases, coordinating with interdisciplinary teams, and providing guidance to clinicians to ensure high-quality, patient-centered care.
