Responsibilities: Conduct comprehensive patient evaluations in home settings and develop individualized PT treatment plans, Deliver therapeutic exercises, manual therapy, and functional mobility training to improve strength, balance, and independence, Educate patients and caregivers on safety, home‑exercise programs, and long‑term self‑management strategies, Coordinate care with nurses, OTs, social workers, and physicians to ensure seamless, interdisciplinary support, Document patient progress, outcomes, and plan updates in the electronic medical record within required timeframes, Maintain HIPAA compliance, safety standards, and equipment sanitation on every visit
Qualifications: Active Arizona Physical Therapist license (Doctorate or Master’s preferred), Minimum 1 year of clinical PT experience (home‑health experience highly desirable), Current CPR/BLS certification, Reliable transportation with auto insurance and valid AZ driver’s license, Strong clinical reasoning, communication, and patient‑education skills, Ability to work independently, manage a dynamic caseload, and adapt to changing patient needs

Day to day: You’ll start each morning by reviewing your patient caseload, route‑planning to maximize visit efficiency. On‑site, you’ll perform evaluations, deliver one‑on‑one therapy sessions (strength, balance, mobility), and educate patients and caregivers on home exercises. Between visits you’ll complete EMR charting, coordinate care with nurses and OTs, and adjust treatment plans based on real‑time patient feedback.
Team: This role reports to the Clinical Manager of Home Health Services, with dotted‑line collaboration to the Regional Director of Therapy. You’ll join a multidisciplinary home‑health team of 20+ clinicians: PTs, OTs, nurses, social workers, plus administrative support. You’ll collaborate daily with one fellow PT mentor and connect regularly with case managers and rehab aides.
