Responsibilities: The Registered Nurse (RN) – Home Health is responsible for delivering comprehensive in-home nursing care, ensuring patients receive personalized and high-quality treatment. This role requires strong clinical skills, excellent communication, and the ability to work independently while coordinating with a multidisciplinary healthcare team. RNs will develop individualized patient care plans, provide education to patients and families, and ensure compliance with healthcare regulations.This is a hybrid role with flexible scheduling options, serving patients in Salem, McMinnville, and Eugene.Patient Care & Treatment• Initiate, revise, and implement individualized patient care plans.• Provide education and counseling to patients and families.• Administer medications, wound care, and other skilled nursing services.• Ensure patient safety and compliance with state and federal healthcare standards.Collaboration & Coordination• Work closely with physicians, therapists, and caregivers to ensure seamless patient care.• Participate in team meetings and in-service education programs to enhance clinical knowledge.Documentation & Compliance• Maintain accurate and timely clinical records in compliance with home health regulations.• Follow state licensure requirements and adhere to best nursing practices.

Reporting Structure: Reports to: Clinical Manager or Director of Rehabilitation Services
Team: Works with a multidisciplinary team including nurses, physical therapists, occupational therapists, and care coordinators., The team is collaborative but also allows for autonomy in managing patient care schedules.
