Interview Process: 1-2 Interviews over zoom with potential onsite. Then two week training period.
Day to day: Plan and carry out ≈ 5–6 in-home visits per day, completing functional ADL evaluations, therapeutic activities, and adaptive-equipment training., Recommend home modifications (grab bars, raised toilet seats, kitchen aids, etc.) that improve safety and independence., Educate patients & caregivers on energy-conservation, joint-protection, and self-care techniques., Document visits in the EMR at point of service and update care plans as goals are met., Communicate progress to the interdisciplinary team and participate in weekly case conferences.
Team: Reports to: Branch Rehab Manager / Clinical Manager (licensed OT or PT)., Alameda branch interdisciplinary mix (approx.): 3 OTs, 5 PTs, 1 SLP, 6 RNs, HHAs, medical social workers, schedulers, and intake coordinators., Follows physician-approved plans of care and CMS home-health guidelines; back-office handles authorizations, QA, and billing so clinicians can focus on patient care.
