Job Overview

We are seeking one full-time Home Health Physical Therapist to join our mission-driven team in Concord, CA. In this field-based role you will deliver compassionate, evidence-based care to patients and their families, focusing on restoring mobility, reducing pain, and promoting independence through tailored therapy plans. You’ll self-schedule approximately 5–6 in-home visits per day, document care in the EMR, and collaborate closely with an interdisciplinary team—all while working in a supportive environment that values collaboration, patient-centered care, and professional growth. The position pays $70 – $85 per hour (DOE), includes mileage reimbursement, and offers a full suite of medical, dental, vision, PTO, and 401(k) benefits. Open to relocating candidates who already hold an active California PT license.

Responsibilities

Key Responsibilities

Evaluate and assess patients' physical conditions and develop individualized treatment plans.
Implement therapeutic exercises and techniques to relieve pain and improve mobility, strength, and coordination.
Educate patients and caregivers on exercises, safety precautions, and self-care strategies.
Collaborate with an interdisciplinary team to provide holistic, patient-centered care.
Maintain accurate and timely documentation of patient progress and treatment plans.

Qualifications

Licensing
- Active Physical Therapist license in practicing state.
Education
- Doctorate or Master’s degree in Physical Therapy.
Experience
- Minimum of 1 year of experience as a Physical Therapist preferred, specifically in home health settings.
Certifications
- Current CPR certification.
Transportation
- Reliable transportation with valid driver’s license and auto insurance.
