Responsibilities: Responsibilities: • Complete an initial assessment of patients and their families to determine home care needs, including a comprehensive physical assessment and review of medical history. • Regularly re-evaluate patient needs and develop a care plan that sets goals based on nursing diagnosis, incorporating therapeutic, preventive, and rehabilitative actions with patient and family involvement. • Initiate the plan of care and make necessary revisions as the patient’s condition and needs change. • Use health assessment data to determine nursing diagnoses. • Initiate appropriate preventive and rehabilitative nursing procedures and administer medications and treatments as prescribed by the physician. • Provide healthcare instructions to patients as outlined in the assessment and care plan. • Identify discharge planning needs and implement them prior to patient discharge. • Prepare clinical notes and update the primary physician as necessary, at least every sixty-two days. • Communicate with the physician regarding any changes in the patient’s condition and obtain/receive orders as required.
Qualifications: • Current and valid CA RN licensure. • Minimum of one year of recent acute care experience in an institutional setting. • Basic Life Support (BLS) certification through the American Heart Association. • Valid driver's license and auto insurance. • Preferred: Bachelor's degree with one or more years of home health care experience.
