The Hospice RN Case Manager is responsible for planning, coordinating, and delivering compassionate hospice care, drawing upon their hospice experience and leadership skills to support patients and their families. Key responsibilities include: Responsibilities: • Plan and coordinate hospice care, utilizing hospice experience to develop individualized care plans tailored to patients' physical, psychological, and social needs. • Lead and supervise hospice aides, ensuring they deliver compassionate and consistent patient care that aligns with hospice standards. • Conduct patient assessments and adjust care plans in collaboration with the Clinical Manager to address evolving patient needs effectively. • Educate patients and families on end-of-life care options, providing compassionate guidance and answering questions based on hospice expertise. • Collaborate with interdisciplinary team members, including physicians, social workers, and hospice aides, to provide holistic, high-quality care.
Qualifications: • Graduate of an accredited school of nursing; current RN licensure in the state.• Minimum of 1-2 years of recent acute care experience in a clinical setting, with 1-2 years of experience in home health or hospice care preferred.• Basic Life Support (BLS) Hands-On CPR certification.• Strong observational, communication, and problem-solving skills.• Valid driver’s license, active auto insurance, and a reliable vehicle, as travel is required for home visits.
