Responsibilities
Responsibilities:
• Plan, organize, and direct hospice care on a NOC (night-off-call) shift, ensuring individualized care plans are developed to meet the diverse needs of patients and their families.
• Coordinate and supervise hospice aides to deliver consistent, high-quality care according to hospice standards.
• Conduct patient assessments and collaborate with the Clinical Manager to adjust care plans as patient needs evolve.
• Provide clear and compassionate education to patients and families regarding end-of-life care options.
• Utilize community resources to support the delivery of holistic care within patients’ homes and communities.
Qualifications
• Graduate of an accredited school of nursing with current RN licensure in the state.
• 1-2 years of recent acute care experience in an institutional setting and 1-2 years of experience in home care is preferred.
• Basic Life Support (BLS) Hands-On CPR certification.
• Excellent observational, verbal, and written communication skills; strong problem-solving and basic math skills.
• Must be able to handle the physical demands of the role including prolonged walking/standing, patient transfers, and lifting supplies/equipment.
• Must be a licensed driver with active auto insurance and a reliable, operational vehicle as travel is required.
• Holistic, complementary, and/or alternative modality training and experience are preferred.
Ideal Candidate
The ideal candidate is experienced, compassionate, and resourceful with a strong background in acute and home care settings. They must be capable of working independently in a traveling role while ensuring consistent, high-quality care is delivered to hospice patients.
