Responsibilities
Responsibilities:
• Plan, organize, and direct hospice care to ensure individualized care plans that address patients’ physical, emotional, and social needs.
• Coordinate patient care for an assigned caseload, including communicating with physicians and interdisciplinary team members.
• Educate patients and family members on self-care techniques, medication management, and end-of-life care options.
• Supervise and mentor hospice aides to ensure compassionate and consistent care delivery.
• Collaborate closely with the broader healthcare team to adjust care plans as patient conditions evolve.
Qualifications
• Graduate of an accredited school of nursing with an active California RN license.
• 1-2 years of recent acute care experience in an institutional setting and 1-2 years of experience in home care or hospice.
• BLS Hands-On CPR certification is required.
• Ability to manage physically demanding tasks (prolonged standing/walking, patient transfers) and perform observational assessments.
• Must possess a valid driver’s license, active auto insurance, and a reliable vehicle for home visits.
