Responsibilities: Conduct comprehensive nursing assessments to evaluate patient and family needs in the home setting., Develop, implement, and regularly update individualized patient care plans in collaboration with patients, families, and the care team., Provide skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management., Educate patients and caregivers on disease processes, treatment plans, medications, and self-care strategies., Coordinate care with physicians, therapists, social workers, and other members of the interdisciplinary team to ensure continuity of care., Monitor patient progress, document clinical findings, and update electronic health records within required timeframes., Identify and plan for discharge needs, ensuring a smooth transition of care.
Qualifications: Current and valid Oregon Registered Nurse (RN) license (Associate’s or Bachelor’s degree in Nursing required; BSN preferred)., Minimum of 1 year of recent acute care or hospital experience; home health experience strongly preferred., Current BLS/CPR certification (hands-on)., Valid driver’s license, auto insurance, and reliable transportation., Strong clinical assessment, critical thinking, and care-planning skills., Excellent communication, documentation, and patient-education abilities., Ability to work independently, manage a dynamic caseload, and adapt to changing patient needs.

Day to day: You’ll start each day by reviewing your patient schedule and planning visits to maximize efficiency. On-site, you’ll perform comprehensive nursing assessments, administer skilled nursing care (wound care, medication management, IV therapy, etc.), and educate patients and caregivers on disease management and self-care. Between visits, you’ll document in the EMR system, coordinate care with physicians, therapists, and social workers, and update care plans based on patient progress and family input.
Team: This role reports to the Clinical Manager of Home Health Services, with dotted‑line collaboration to the Regional Director of Therapy. You’ll join a multidisciplinary home‑health team of 20+ clinicians: PTs, OTs, nurses, social workers, plus administrative support. You’ll collaborate daily with one fellow PT mentor and connect regularly with case managers and rehab aides.
