Interview Process: 1-2 Interviews over zoom with potential onsite. Then two week training period.
Day to day: Plan and carry out ≈ 5–6 home visits per day, performing evaluations, therapeutic exercise, gait training, and caregiver education., Collaborate daily with nursing and therapy colleagues to ensure seamless, patient-centered care., Document visits in the EMR at point of service and participate in weekly interdisciplinary case reviews., Educate patients and families on safe mobility, home-exercise programs, and fall-prevention strategies
Team: Reports to: Branch Rehab Manager/Clinical Manager (licensed PT)., Works under physician-approved plans of care and within CMS home-health guidelines for clinical oversight., Administrative support (authorization, QA, billing) handled by the regional back-office so clinicians can focus on patient care.
