Responsibilities: Perform comprehensive in‑home patient evaluations and develop tailored PT treatment plans, Deliver therapeutic exercises, manual therapy, and functional mobility training, Educate patients and caregivers on safe home‑exercise programs and self‑care strategies, Coordinate interdisciplinary care with nurses, OTs, social workers, and physicians, Document all patient interactions, progress notes, and plan updates in the EMR promptly, Maintain HIPAA compliance and uphold safety and sanitation standards on every visit
Qualifications: Active Arizona Physical Therapist license (Doctorate or Master’s preferred), Minimum 1 year of clinical PT experience (home‑health experience highly desirable), Current CPR/BLS certification, Reliable transportation with valid AZ driver’s license and auto insurance, Authorization to work in the U.S., Strong clinical reasoning, patient‑education, and documentation skills, Ability to work independently and manage a travel‑intensive caseload effectively

Interview Process: Phone Screen (20–30 min): Recruiter will confirm your Arizona PT license, CPR/BLS status, home‑health experience, and logistical fit., Hiring Manager Interview (30–45 min): Clinical Manager will dive into your clinical reasoning, patient‑education approach, and case examples., Skills Demonstration (in‑home or virtual; 60 min): You’ll present a brief treatment plan and demonstrate hands‑on or virtual techniques with a mock patient scenario., Final Leadership Review (15–20 min): Regional Therapy Director discusses role expectations, career path, and answers any remaining questions.
Day to day: Each morning you’ll review your scheduled visits, map out the most efficient route across Sun City and surrounding areas, then spend your day performing patient evaluations, delivering one‑on‑one PT sessions (therapeutic exercise, balance and gait training, manual techniques), and educating patients and caregivers. Between visits you’ll complete EMR documentation, coordinate with nurses and OTs, restock supplies, and update treatment plans based on patient progress.
Challenges: Navigating varied home environments (stairs, tight spaces) and adjusting treatment accordingly, Managing drive times, traffic, and weather across multiple service areas, Balancing thorough, compliant charting with maximizing patient contact time, Adapting care plans quickly when patients’ conditions or home situations change
Reporting Structure: Reports directly to the Clinical Manager of Home Health Services, with dotted‑line collaboration to the Regional Director of Therapy.
Team: You’ll be part of a 20‑member home‑health team—including PTs, OTs, nurses, social workers, and rehab aides—supported by administrative coordinators. You’ll have a dedicated PT mentor and regularly liaise with case managers and interdisciplinary clinicians.
Growth: Clear advancement paths from staff PT to lead PT, clinical educator, and regional therapy manager. We offer CEU stipends, leadership workshops, and structured mentorship to support your professional development.
