Day to day: Each morning you’ll review your scheduled visits, map out the most efficient route across Sun City and surrounding areas, then spend your day performing patient evaluations, delivering one‑on‑one PT sessions (therapeutic exercise, balance and gait training, manual techniques), and educating patients and caregivers. Between visits you’ll complete EMR documentation, coordinate with nurses and OTs, restock supplies, and update treatment plans based on patient progress.
Team: Reports directly to the Clinical Manager of Home Health Services, with dotted‑line collaboration to the Regional Director of Therapy. You’ll be part of a 20‑member home‑health team—including PTs, OTs, nurses, social workers, and rehab aides—supported by administrative coordinators. You’ll have a dedicated PT mentor and regularly liaise with case managers and interdisciplinary clinicians.
