The Occupational Therapist (OT) – Home Health role involves delivering high-quality, in-home occupational therapy services to patients in East Portland and Gresham. This position is a Pay Per Visit role with an average earning potential of $93,000–$98,000 per year, offering flexibility, patient-centered care, and collaboration with interdisciplinary team members.
Responsibilities
Conduct Comprehensive Assessments: Perform detailed assessments of each patient’s functional status, including physical, psychological, and social aspects, to develop a holistic view of care needs.
Develop Treatment Plans: Create individualized treatment plans and set measurable goals that support patients’ unique needs and abilities, adjusting as necessary to facilitate progress.
Educate and Empower Patients: Guide patients in regaining lost skills or adapting to new ways of completing tasks, offering practical strategies and tools for improving their daily functions.
Collaborate with Interdisciplinary Teams: Work alongside other healthcare professionals to ensure a coordinated, holistic approach to patient care.
Record Keeping: Maintain accurate and timely clinical records, including detailed notes on patient progress and treatment effectiveness, to support continuity of care.
Qualifications
Licensure: Valid Occupational Therapist license in Oregon.
Driver’s License and Vehicle: Valid driver’s license, reliable transportation, and appropriate insurance for travel to patient homes in the coverage area.
Skills: Excellent communication and interpersonal skills to work effectively with patients and team members.
Work Style: Ability to work independently in an autonomous setting, demonstrating strong time-management and organizational skills.
