This Physical Therapist (PT) role seeks a compassionate and skilled clinician with at least one year of experience, ideally in home health, to deliver personalized in-home care. Responsibilities

As a Physical Therapist, your role will include:

  • Delivering physical therapy services according to physician's plan of care.

  • Assisting patients with exercises, gait training, and prosthetic training.
  • Maintaining clinical records and documentation in a timely manner.
  • Participating in team meetings and in-service education programs to enhance skills and knowledge.


Qualifications

  • Licensure: Licensed Physical Therapist in California.
  • Education: Doctorate or Master’s degree in Physical Therapy.
  • Experience: Minimum of one year of PT experience required.
  • Certification: Current CPR certification.
  • Other: Valid driver’s license, insured vehicle in good working order

Reporting Structure

This role will report to the Clinical Manager or Director of Rehabilitation Services. They will be supervised by the leadership team while maintaining autonomy in their patient care and scheduling.
