Job Overview

We are seeking one full-time Home Health Occupational Therapist to join our mission-driven team in Alameda, CA. In this field-based role you will help patients regain independence in activities of daily living by delivering evidence-based OT interventions, adaptive-equipment training, and caregiver education. You’ll self-schedule roughly 5–6 in-home visits per day, document care in the EMR, and collaborate closely with an interdisciplinary team—all while working in a supportive environment that values collaboration, patient-centered care, and professional growth. The position pays $70 – $85 per hour (DOE), includes mileage reimbursement, and offers a full suite of medical, dental, vision, PTO, and 401(k) benefits. Open to relocating candidates who already hold an active California OT license.

Responsibilities

• Evaluate functional abilities in the home

• Implement ADL-focused therapeutic activities (dressing, bathing, cooking)

• Recommend/adapt equipment for safety & independence

• Update care plans and document in EMR

Qualifications

  • Licensing
    • Active OT Therapist license in practicing state.
  • Education
    • OT degree (OTD/MOT/BSOT)
  • Certifications
    • Current CPR certification.
  • Transportation
    • Reliable transportation with valid driver’s license and auto insurance.

Must-Have Requirements

  • Active Occupational Therapist (OT) license in the state of California
  • Doctorate, Master's, or Bachelor's degree in Occupational Therapy (OTD, MOT, or BSOT)
  • Minimum of 1 year of experience as an Occupational Therapist, ideally in home health
  • Current CPR certification
  • Reliable transportation with a valid driver’s license and auto insurance
  • Ability to cover up to a 50-mile radius for home visits
  • Strong documentation skills and familiarity with EMR systems
